Insurance Information
As a courtesy to you, we are happy to submit your claim to your dental insurance carrier. We ask that you provide us with all of your insurance information at the time you schedule your appointment. We will then contact your insurance carrier to find out your specific plan benefits, and let you know your estimated portion prior to your appointment.
Be advised that coverage verification does not guarantee payment by your insurance company. While we try our best to estimate your out-of-pocket expense, actual payment is determined by the insurance company once they process the claim.
At the time of service, you will be responsible for the estimated portion that your insurance will not cover. Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage. We will wait 90 days for the insurance company to pay their portion. In the event your insurance company fails to pay, rejects the claim, or does not pay the total balance, you are responsible for the remaining balance.
If your insurance pays more than the estimated amount, a refund check will be mailed to your home address. if your insurance pays less than the estimated amount, you will receive a statement from our office. Your prompt remittance is greatly appreciated.
Currently, we are contracted providers for Delta Dental, Dental Health Alliance (DHA), most Aetna plans, Assurant, Regence Blue Cross Blue Shield of Oregon and DeCare Dental (Teamsters).
